What's changing?

The SNAPs field is being added into the Neo Transition Plan, effective August 28th, 2023.  The SNAP is currently in the SPM Program Termination/Discharge Summary and with this change, it it's done in the Transition Plan, it will pre-populate into the Discharge Summary, so there won't be duplication (unless something changes).
