This list will include the current assigned Care Coordinator and specific programs that the client is open to with their assigned provider (if applicable).
| ℹ️This information is taken from the SPM section in Neo. Once the client is assigned to a program/staff through SPM, it won't immediately appear in the Care Team screen - give it a little time before you expect to see it there. |
Care Coordinator
Care coordinators can be easily changed through the Care Team screen. The drop-down menu will include care coordinators and any KBH staff assigned to the client.

..will reveal a drop-down menu with a list that includes care coordinators and any KBH staff assigned to the client.

Select the new care coordinator from the drop-down menu.
Save.
🗹 The new care coordinator will now be listed as a care team member for KBH.
