Updating A Treatment Plan

  • Per contract and licensing regulations, a Treatment Plan for CSU stays must be entered within 24 hours of an admission and then reviewed and updated at the seven(7) day mark and then again, every two(2) days thereafter.
  • When creating a Treatment Plan, set the end date for the 7th day of admission. Updated plans after that seven(7) days should be set for two(2) days for each plan thereafter.
  • Updating Treatment Plans after seven(7) days is similar to the initial plan.

 

Initial Seven(7) Day Plan

Log in to Neo and navigate to the Treatment Plan module.

You'll find Treatment Plans in the Client Menu in the Information category - you can also pin it to your favorites for quicker access.

Select the CSU program

Click Add New

For the initial plan, set the date out seven(7) days.


 

Updated Plans

Log in to Neo and navigate to the Treatment Plan module.

You'll find Treatment Plans in the Client Menu in the Information category - you can also pin it to your favorites for quicker access.

Select the CSU program

Click Add New

For the initial plan, set the date out seven(7) days.

When you add a new plan at the seven(7) day mark, it will carry forward the Goals and Action Steps that were previously identified in the initial plan.

Extend the anticipated completion dates by two days by selecting the edit button next to each Goal/Action Step.

After selecting Edit, the following screen will pop up, and you can either extend the completion date, mark the Goal/Action Step as met, or close it.

If you are still going to work on the goal, extend it by two(2) days. Click Save after you’ve made edits.

If there are additional Goals/Action Steps to add to the plan, select Add ST Goal or Add Action Step:


 

Rationale for Continuing Care

This box is for providing an explanation around the factors for why a client needs to extend their stay and should demonstrate medical necessity.

Examples include: 

  • Frequency/intensity of suicidal ideation
  • Safe  discharge plan not available for another 24 hours
  • Medication concerns exist, and a provider appointment is scheduled for the following day, etc.

Description of Client Progress

At Admission:  It would be acceptable to note here that this is the initial Treatment Plan, and so there has been no progress to remark upon.

For Extended Plans:  If we are extending out a Treatment Plan, then the description of progress should remark on each goal and whether or not they have achieved the goal, and if not, what were the barriers to success.
