    Updating A Primary DSM Through An Addendum

    You'll find the option to update a DSM through the Addendum tool on most Crisis notes in Neo.

    Crisis Managers and Team Leaders are the staff with the security role that allows this kind of Addendum.


     

    Why would you update a DSM through an Addendum?

    • At the time the note was created, there was not an active DSM on file.  
    • DSM existed at the time the note was created/completed/signed, but you want to change which diagnosis is designated as Primary.

    When is an Addendum allowed?



    Use the Primary DSM dropdown menu to select the diagnosis that you're updating to.

    *The dropdown menu will only include diagnoses that are listed on the current/active DSM.

    If a DSM was never completed, there will be no Current Primary DSM listed, and no DSM options listed in the dropdown menu.


     

    You'll see the Current Primary DSM listed beside the Primary DSM field.


     

    Once the Addendum has been saved, clicking Addendum as if you were adding another Addendum will show you a record of the change you just made.  The updated primary DSM will also now reflect in the Current Primary DSM field.

     

    The update also shows in the PDF of the note, as shown below:
