A signed Release of Information (ROI) is required in order to allow KBH to share/release your information in any capacity.
The ability to create and obtain a client signature on a release using the Staff App is intended to be an extra tool for obtaining the document conveniently and instantaneously.
For that reason, there are some restrictions on releases created using the Staff App compared to creating a release in Neo or filling one out on paper.
- The Organization you are intending to create a release for from the app, must already exist in the Neo database.
- Organization name and contact information, and sections A, B C, E, F and G are all pre-filled and cannot be changed
- Section (A) The period for which information is requested is From - this populates with the start date of the current episode of care and expires one month from the date of creation/signature.
- Section (B) All options checked off, including Other with “Any information needed for coordination of care” specified.
- Section (C) Coordination of Services option checked off.
- While Section (D) is also pre-selected, these options can be changed, based on the client's preference.
- Section (F) Checked off.
- Section (G) Pre-filled with a one month expiration date.
If all of those selections are appropriate for what you are needing the release for, follow the steps below to complete the process. |
Select your client.
From the client screen, click Add Coordination Release.
Use the dropdown menu to select an organization.
The available options for Organizations
Please contact Medical Records to have the organization added to our list. In the meantime, you can use Neo to create the release - print for client signature or send it to the Client Portal for the client to sign via the Staff App or from the Client Portal. |
Verify that the contact info is accurate and click Add Release.
Select the signer or manually enter them into the Signer field and click Continue .
The client can change section D if they so choose, but otherwise just needs to sign in the signature box at the bottom of the page and click Complete Document
The Coordination Release is immediately active in the client’s chart and available to be viewed right from the app.
