The Patient Health Questionnaire (PHQ-9) can be completed via:
- Neo (from the PHQ-9 screen or on paper and then scanned via Medical Records)
- Client Portal
- Staff App

Completing in Neo
If you are completing the PHQ-9 with the client, face-to-face, there are two (2) ways to access the screening.
Clicking the red, PHQ-9(Adult) button in the Presenting Problems section of the Assessment will generate a new instance of the screening and will load in a separate window right from the screen you're in. ![]() |

Locate the PHQ-9(Adult) screening in the Client Menu. Â You will find this listed under the Screenings category of the CCBHC section of the Client Menu. Â The same is listed under the Screening section of the Clinical category. Â You can click the pushpin beside the menu item to pin it to your favorites for quicker access. | |

 Clicking Add PHQ-9(Adult) from this screen will generate a new instance of the screening and will load in a separate window right from the screen you're in. ![]() |

Completing through the Client Portal/Staff App
The PHQ-9 (Adult) screening can be completed electronically using either the Client Portal or the Staff App. Â This will require you to send out the form electronically via Document Delivery. Â
