CSP Follow Up Questionnaire Due Aligns with the authorization and when it is due for an extension. OR 90 days from Single Assessment if no Case Management authorization in place. |

On the Alerts page, you'll find these alerts listed under Coordinated Service Plan (CSP) Alerts.   |
If this alert isn't showing up on your list (other than when there isn't anything due), click here to learn how to add it to your list of alerts. |

Clicking the S beside each alert, will bring you directly to the screen in Neo where you'll be completing the required task.  |
Needs Single Assessment Date entered

Click Add.  |
|
Add the begin date of the assessment and select Single Assessment Date from the Type dropdown menu.  |

CSP Questionnaire due

Click Add.  |
|
Complete the CSP Questionnaire and click Save.  |

Once the questionnaire is complete, enter your Electronic Signature and click Sign.  |

Once the CSP Questionnaire has been completed, it will populate in the CSP Questionnaire section of the MaineCare P.A. 
This questionnaire will still appear in this section whether or not one has been completed. The fields will always be read-only (greyed out). You'll need to go into the CSP screen separately to complete this. If the authorization is created before the CSP, click the Reload button to pull it into the MaineCare P.A.
CCBHC auths. are automated and will attempt to upload regardless of whether the CSP is completed. If CSP is due for this client (based on single assessment date) but CSP is not completed before the authorization is due, the authorization will fail to upload due to the questionnaire being incomplete. |
