    Submitting a Care Coordinator Request (CCR)

    Learn where to submit a Care Coordinator Request.


     

    From the Client Menu

    You'll find Care Coordinator Requests in the Client Menu in the CCBHC category - you can also pin it to your favorites for quicker access.

     

    Click Add to start a new Care Coordinator Request.

     

    Enter the request information and Save.  Change the Status to Ready for care coordinator, to let Care Coordinators know this request is ready to be worked on.


     

    From Quick Links

    You'll find the Quick Links menu in the bottom left-hand corner of your screen when you're in the client account area of Neo.

     

     

     

     

     

    Please Note:  Care Coordinator Requests will not appear on your Quick Links menu unless you adjust your settings.  Click here to learn how to adjust your Quick Links settings.

     

     

     

     

     

     

     

    Click Add to start a new Care Coordinator Request.

     

    Enter the request information and Save.  Change the Status to Ready for care coordinator, to let Care Coordinators know this request is ready to be worked on.


     

    From the CCBHC Dashboard

    Click the CCR button to start a new Care Coordinator Request (CCR).

     

    Enter the request information in the request field.

     

    • Clicking Submit to CC will submit the request with a Ready for care coordinator status.
    • Clicking Save will save the request with a Not sent to care coordinator status.  It will show as Incomplete in the Care Coordinator Requests area in the Client Menu.

     

     


     

     

    Once you select the Ready for care coordinator status, it will reflect on the Care Coordinator Requests Dashboard for a Care Coordinator to begin working on the request.

     

    Note:  Once you change/Save to Request closed, no action needed, it cannot be changed again.
