- An SPM is needed when:
- A client would like a referral for additional services within KBH (Referral)
- A client would like to continue with school-based services but needs to be transferred to a different clinician for any reason or move from a school-based program to another KBH program (Transfer)
- A client needs to be closed to school-based services (Program Termination)
- You need to end services with a client but keep them open to the school-based program (Provider Termination)
- To start an SPM, open the client's chart in Neo and under the Client Menu click SPM - you should see a screen that looks like this:
Notice the tabs along the top – Referral, Transfer, Program Termination, Provider Termination. Each tab has a different purpose, which is stated above the tabs in Green (see step 1)
3. Click on the tab needed and a new screen will pop up. The form will look slightly different based on which option you choose. Use the drop-down menus to complete the form. Notice the gray sections, these sections prepopulate for you. (Double-check you have the correct client.)

This drop down doesn’t have to be filled out unless the client is requesting a specific provider they would like to work with. If it is left blank the program will decide and assign the client to who they feel is an appropriate fit. |

You have now completed the SPM for a referral to another program. Sign the SPM using your electronic signature. This will send the SPM to your supervisor for approval. If it’s time-sensitive, email your supervisor to alert them of the pending SPM.
