Many times, you'll run into the scenario where a client already has a designated PCP in Neo but is either in the process or has already established care with a new PCP. While only one PCP can exist in the Care Team screen at one time, you'll still want to initiate the process of replacing it, by getting the new PCP added into Neo as an External Care Team until the client has been established as a patient there, and we've received a valid release of information.
| In the following scenario, this client's PCP is Winthrop Pediatric & Adolescent Medicine but they've reportedly established care with Elmwood Primary Care. You don't want to replace the previous PCP until you coordinate and get a release on file. |
Winthrop Pediatric & Adolescent Medicine is listed as the client's Primary Care Provider(PCP).
Add the new PCP but list the Care Team Type as External Care Team for the time being - you can also make a note that this will be client's new PCP.
It will be listed under External Teams for the time being.
Once the client has been established with the new PCP and a valid release is on file, make it their PCP in Neo by editing the care team, and changing the Care Team Type to Client PCP.
If you've made a note that is no longer relevant, delete it at this time.
The new PCP Elmwood Primary Care is now listed as the client's PCP. The previous PCP will be end dated for the day before the new PCP begins.
