Referrer Profile
Use your Referrer Profile to indicate whether or not you work for DHHS, a PCP office, or which specific organization you are associated with.

Access your profile by clicking on your email address, listed at the top right-hand side of the screen once you've signed in.  |

Here's an example of what a Referrer Profile looks like.
Please note: if you make any changes in your profile, you will need to sign out and then back in again for the changes to reflect in your account.  |

Update your Name or Email Address, if needed.  |

Do you work for DHHS? Select (Not with DHHS) or select which DHHS location you are associated with.  |

Do you work for a Primary Care Organization? Select (Not with PCP) or select which PCP organization you are associated with.  |
|
Do you work for a different organization? Select (Not with any specific Referral Source) or select which organization you are associated with.  |
|
If you don't see your organization listed, use the text field to add the name of your organization, its address, phone number, etc.  |
