General Referral
A General (Individual) Referral is a referral to KBH for yourself or on behalf of a family member. You can submit a referral with or without signing up for an account.
The following instructions are for submitting a General referral without signing up/signing in.

Complete all fields of the referral. There is a space to upload documents at the end of the form. If any fields are missed or incomplete, you will be alerted when you attempt to Complete the referral.  |

There is a minimum amount of information that is required before it will allow you to submit, so you will see an alert like the one below, if you've missed any of those fields.  |

Once you click Complete, your referral will be submitted to the KBH, and you will see confirmation on your screen including which phone number you should use if you would like to contact us regarding your referral.
