General Referral
A General(Individual) Referral is a referral to KBH for yourself or on behalf of a family member. You can submit a referral with or without signing up for an account.
Signing up for an account allows you to save your place and get back to a referral date and/or track the status of your referral over time.

To submit a General Referral, click Get Started.  |

To begin a new referral, click the blue Add New Referral button.  |

Complete all fields of the referral. There is a space to upload documents at the end of the form. If any fields are missed or incomplete, you will alerted when you attempt to Save/Submit the referral.  |

If you click Save the status of your referral will be Incomplete.  |

Clicking Submit will submit the referral to the agency.  |

Once you click Complete, your referral will be submitted to the KBH and you will see confirmation on your screen including which phone number you should use if you would like to contact us regarding your referral.  |
