Requirements for State CCBHC Demonstration and CCBHC IA Grant
CCBHC Screenings
Effective Date: July 1, 2025
- Kennebec County CCBHC – Required Screening Process
Initial Evaluation
At the time of the Initial Evaluation, the following screenings will be completed for all new CCBHC enrollees by Access Center staff:
- C-SSRS (Columbia-Suicide Severity Rating Scale)
- PRAPARE (Protocol for Responding to and Assessing Patients’ Assets, Risks, and Experiences)
- PHQ-9 (Patient Health Questionnaire-9)
- AUDIT (Alcohol Use Disorders Identification Test)
Annual Assessment
- A PHQ-9 will be required as part of the Annual Assessment for all CCBHC enrollees.
- The PRAPARE and AUDIT screenings will become required components of the Annual Assessment beginning January 1, 2026.

Initial Evaluation
At the time of the Initial Evaluation, the following screenings will be completed for all new CCBHC enrollees:

- PHQ-9
- AUDIT
Annual Assessment
- A PHQ-9 will be required as part of the Annual Assessment for all CCBHC enrollees.
- The PRAPARE and AUDIT screenings will become required components of the Annual Assessment beginning January 1, 2026, except for clients identified as part of the IA Grant cohort.
- IA Grant Cohort Exception
For clients identified as part of the IA Grant cohort:
- Both the PHQ-9 and the AUDIT are required as part of the Annual Assessment.
- Care Coordinators and Case Managers will complete the PRAPARE within the Annual Assessment timeframe.
- A PHQ-9 must also be completed when specific clinical indicators are met. In these instances, an alert will appear in Neo prompting completion.
